Your First Appointment

In order for us to better serve your medical needs, we have supplied our Patient Information Form and our Health Information Form. If your insurance requires a referral please call your primary care physician and request one.

What to bring with you on your first visit:

• Insurance Cards
• Co-pay (we accept cash, check, VISA, MC, and Discover)
• Referral
• List of your medications/allergies
• If possible results of Lab work / X-rays pertaining to your visit with us
• The following forms filled out:
